الدراسة اعتمدت على ملاحظات المرضى عن نوبات صداعهم لمدة 28 قبل التجربة باعتبارها نقطة الأساس للمقارنة -العلاج الفعال ما يقلل النوبات بمقدار النصف -
candesartan(4-32) Vs topiramate(25-100)
عدد المرضى فى التجربة كان كالاتي
462 receiving candesartan Vs 199 receiving topiramate
prim analysis : discontinuation rate with in 6 m
sec analysis :efficacy
results
- candesartan provided a greater reduction in monthly headache days (MHD) compared to topiramate (-4.5 vs. -3.1 days, p = 0.04)
- No significant differences were observed No significant differences were observed between candesartan and topiramate regarding reductions in monthly migraine days (MMD), monthly acute medication days (MAMD), or Headache Impact Test (HIT-6) scores
التحليل الدقيق للدراسة يظهر معايب مانعة من اعتبارها سواء لحقيقة ان المعامل المذكور معدل حسابيا لا يوافق الحقيقة

تنبيهات ضرورية
1-never use carbamazepine as preventive therapy for migraine " it is used for trigeminal neuralgia , the only accepted condition to try it is "neuralgia induced migraine , the chronic form of trigeminal induced migraine attacks"
2- evidence supporting use of CCB for migraine is very poor
"it is more reasonable to try candesartan in hypertensive patients "
verapamil is often used in practice (not an established EBM" HOWEVER Lomerizine is the most used medication for migraine prophylaxis in Japan.
