Golden diagnostic rule "the expert level"
(1) if the primary tissue site won't clear unlike the bacteremia , look for a micro-niche interaction (eg lung abscess)
(2) If the primary tissue site is resolving but the blood remains persistently positive for MRSA, look for a distal macro-systemic interactive ecosystem (eg MRSA pneumonia +klebsiella /pseudomonas UTI , two separate sites but interactive bacteremia)
