Sometimes, Life Gives You One More Beat!
A remarkable story of survival against the odds at Gode General Hospital in the Somali Region of Ethiopia, proving that swift action, clinical dedication, and teamwork save lives even in resource-limited settings.
A 40-year-old man presented to our Emergency Room with severe, squeezing left-sided chest pain radiating to his arm and neck. Having initially mistaken the pain for dyspepsia, he had tried to sleep it off before seeking care.
Upon arrival, he walked in hemodynamically stable, but his ECG showed ongoing myocardial ischemia, raising immediate concern for Acute Coronary Syndrome (ACS). Aspirin was loaded, and troponin levels were ordered.
Then, in an instant, the situation turned critical.
The patient suffered sudden cardiac arrest becoming completely unresponsive and pulseless. The monitor displayed Ventricular Fibrillation (VF).
Our resuscitation protocol was initiated immediately:
Shock → CPR → Shock → CPR
Airway Management: Following the second shock, the patient was successfully intubated during ongoing CPR to support oxygenation.
Medication: After the third shock, targeted antiarrhythmic therapy was administered.
Despite facing refractory VF far removed from tertiary cardiac centers, the emergency team persisted. After five relentless cycles of CPR and defibrillation, their determination paid off: ROSC was achieved.
He was transferred to the ICU for mechanical ventilation and vasopressor support, embarking on an incredible recovery trajectory:
Day 3: Successfully extubated.
Day 5: Formal echocardiography performed by our cardiologist revealed anteroseptal hypokinesia, confirming the ischemic etiology.
Day 7: Transferred to the medical ward after thorough counseling with the patient and his family regarding the necessity of further evaluation, including coronary angiography.
Day 8: The very same man who arrived in cardiac arrest and refractory VF walked out of the hospital alive.
