▫️شنو اسباب af ؟
طبعا اكو شي اسمه risk factors واكو causes بس هي كلها theory لسبب af يعني نظريات
ناخذ مثال هذا المريض عنده ضغط وسكر ويدخن وذني كلهن يعتبرن risk factors
اصلا الضغط وحده اكثر سبب يسوي af بمعظم الدول
▫️شنو ال risk factors ؟
تسال المريض بالهستري
اول شي عمر المريض
👉🏻Advanced age
👉🏻Pmh( dm and HT )
👉🏻Social hx ( smoking and alcohol)
واذا جان المريض يعاني من السمنة و obstructive sleep apnea

اسباب af يقسموها الى:
🔹Cardiac causes :
👉🏻coronary artery disease
وهذا اكثر سبب نشوفه بالطوارئ هو وفشل القلب
👉🏻heart failure
👉🏻valvular heart disease ( mitral disease)
هاي كلش مهمة لذلك ممكن يكون عدهم murmurs اهل af مثل mitral stenosis or regurgitation ثنينهم يسون af
👉🏻cardiomyopathy
👉🏻pericarditis ( ممكن تسوي😲)
👉🏻sick sinus syndrome
👉🏻wpws
📌 ليش لازم نعرف الاسباب؟ لان مرات حتى لو عالجت af ما يفيد اله تعالج السبب.
🔹Noncardiac causes :
تشمل هواي امراض
👉🏻pulmonary disease ( copd, pneumonia,pulmonary embolism)
مريض copd وما جاي يتحسن ممكن عنده arrhythmia وهم مرضى pulmonary embolism مرات نشوف بecg af
👉🏻thyrotoxicosis or pheochromacytoma
امراض الغدة الدرقية
👉🏻electrolytes disturbances ( k, mg)
لذلك بالتحليل ندز TFT and s electrolytes
👉🏻drugs (digoxin, adenosine, amphetamine, cocaine,…)
نسال بالهستري اذا ياخذ ادوية او مدمن مخدرات او يشرب كحول
👉🏻increase sympathetic activity
هذا السبب يشمل كل سبب يزود sympathetic يعني يزيد adrenaline and noreadrenaline
مثل شنو؟
👉🏻sepsis
👉🏻hypovolemia
👉🏻post surgery
👉🏻hypothermia
*Approx. 15% of individuals who develop Afib have none of the above mentioned risk factors (idiopathic/lone Afib)
مرات مريض ما عنده كلشي ولا يدخن ولا ياخذ ادوية ولا امراض وشاب ممكن يصير بي؟
اي ممكن ويسموها lone af يعني بدون سبب
🔶Examination of patient with Atrial fibrillation:
⁃ Look for signs of thyrotoxicosis : goiter , staring look , fine tremor, wt loss ..
⁃ Check blood pressure: hypertension most common cause of AF
⁃ Face : Malar flush : mitral stenosis , parotid enlargement: alcoholic?
⁃ Cardiac : murmur: Mitral stenosis or regurgitation , wide spliting of S2: ASD? , pericardial friction rub : pericarditis , sternotomy : previous cardiac surgery?
⁃ Respiratory findings of pneumonia, COPD : can complicated by AF
⁃ Abdominal: hepatosplenomegaly and jaundice: alcoholic?
⁃ Raised JVP , LL edema : heart failure common cause of AF
⁃ Unilateral limb swelling: pulmonary embolism complicated by AF?
Just remember the causes !
