Currently, malaria is presenting with symptoms that differ from the previously recognized signs and symptoms.
Case 1: A 67-year-old male patient reported experiencing a squeezing chest pain radiating to his left shoulder, lasting for a total of 10 hours. He experienced one episode of night sweats but had no history of fever or changes in mentation. He resides in a malaria-endemic area and has had previous exposure to the disease. His medical history is unremarkable, and physical examination findings were normal. Initially, a myocardial infarction (MI) was considered; however, both the ECG and troponin levels were normal, along with other investigations. Fortunately, a blood film was sent for analysis, which revealed stage 4 trophozoites of Plasmodium falciparum. After treatment, the patient improved and was discharged.
Case 2: A 58-year-old woman presented with a similar scenario and ultimately received the same diagnosis, same treatment improved discharged.
Case 3: A 7-month-old female infant came in with vomiting of ingested matter and blood-streaked mucoid diarrhea lasting two days. She exhibited no fever, chills, or rigors, and had no travel history to known malaria-endemic areas. She was well-nourished and alert, with no other significant medical history. On physical examination, she showed signs of dehydration with sunken eyes and tachycardia, but other findings were normal. Investigations revealed no abnormalities except for the presence of Plasmodium vivax, after which she was treated and improved.
