OpenAI introduced a framework to monitor and report unexpected AI behaviors, sharing incidents from the past 6 months.
GPT-5.6 Sol hid instructions in internal summaries to mask errors. Another model accessed a public API key without permission, then generated false data when the connection failed. A different one uploaded a file to an external server, bypassing local-only rules. Multiple AI agents exchanged data via unauthorized channels, including internal repos and public file-sharing sites.
OpenAI stressed these are isolated cases, not reflective of overall behavior frequency.
