Between a CRISPR injection that could kill the $500K price tag and an AI vaccine hitting 75% response rates, the economics of cancer treatment are about to break open.

The biggest story is a Nature paper from UCSF that could reshape cancer treatment economics. Researchers used CRISPR nanoparticles to create CAR-T cells directly inside the body — no extraction, no lab, no weeks-long manufacturing. A single injection cured leukemia and myeloma in mice within two weeks, and the in vivo-created cells actually outperformed lab-made ones. Current CAR-T therapy costs $400-500K per patient. If this translates to humans, it drops to the cost of an injection.

AI is accelerating the other bottleneck — picking which tumor mutations to target. At AACR 2026, Evaxion's AI-designed neoantigen vaccine EVX-01 achieved a 75% objective response rate in advanced melanoma combined with pembrolizumab, with 92% of responders still responding at 24 months. The platform predicted which neoantigens would be immunogenic — and 84% of them actually triggered immune responses. Separately, circular RNA is emerging as a superior delivery format: AACR data showed circRNA-encoded neoantigens produced stronger T-cell killing than linear mRNA, signaling a generational shift in vaccine technology.

Meanwhile, drugs already prescribed to millions are showing unexpected anti-cancer effects. A JAMA review covering 1.65 million diabetic patients found that GLP-1 agonists like semaglutide cut pancreatic cancer risk by 59%, liver cancer by 53%, and colorectal cancer by 46%. No RCT has confirmed causation yet, but if it holds, the world's most popular obesity drugs are accidentally running the largest cancer prevention trial in history.
