من يومين شوفت حالة فى المستشفى
18 yrs old, female pt diagnosed with Wilson's disease , presented with severe jaundice, anemia and vaginal bleeding for 1 wk , she has 1ry amenorrhea .
The pt needs liver transplant urgently، her sister has also been diagnosed with WD
الأم فى الأساس جاية علشان ال vaginal bleeding
كل التفكير وقتها كان هل دى
period or just vaginal bleeding as a manifestation of bleeding diathesis due to liver cirrhosis ?🤔
اتعرضت تروبيكال و الدكاترة ابتدوا ال assessment عادى بس اللى شاغل بالى كان هل Wilson's dis له علاقة وثيقة بموضوع ال 1ry amenorrhea وممكن تكون هى ال first presentation ولا دى صدفة ؟
( أول مرة أشوف حالة Wilson ومش ملمة بكل المعلومات اكيد )
بعد بحث طويل فى journals مختلفة وبمساعدة chat gpt
ظهر أسباب كتير ممكن توضح علاقة زى دى أولها كان ال
1. Hepatic Dysfunction:
Chronic liver disease in Wilson’s can disrupt the hypothalamic-pituitary-ovarian (HPO) axis, leading to hormonal imbalances that can delay puberty or cause amenorrhea.
ودا يمكن الأوقع فى الحالة اللى معانا
Liver dysfunction affects metabolism of sex hormones like estrogen and progesterone.
لأن الكبد هو the main site for estrogen and progesterone metabolism وبالتالى بال negative feedback ممكن تحصل amenorrhea
2. Hypogonadotropic Hypogonadism:
طبيعة ال Wilson أنه عبارة عن copper accumulation in various body tissues mainly liver and brain وعلشان كدا اسمه hepato lenticular disease ودا اللى بيخلى اهم أعراضه بتكون hepatic and neuropsychiatric
وبالتالى
Copper accumulation in the brain may affect the hypothalamus or pituitary, suppressing gonadotropin-releasing hormone (GnRH) and downstream LH/FSH secretion.
وبنفس الطريقة ممكن يترسب فى المبيض ويعمل
Direct Ovarian Toxicity
3. Nutritional Deficiency and Chronic Illness:
Severe disease can result in poor nutrition and chronic stress on the body, both of which can delay menarche.
طب لو كل دى اسباب لل 1ry amenorrhea in WD females
يا ترى اى سبب ال vaginal bleeding
وللعلم فى سبب تانى كان مكتوب فى study أنه حالات كتير من ال WD بيكون معاهم pco as endocrine comorbidity causing anovulation so they recommended routine screening for pco in adolescent Wilson's dis pts
بس دا كله ميخليناش ننسى أنه أول حاجة زى ما دكاترتنا علمونا لازم نستبعد ال
local causes : vaginal or cervical polyps , foreign body
ال polyps تستبعد بدري شوية لانها مش common in this age group
سبب تانى ممكن يكون
- Estrogen Breakthrough Bleeding
Anovulation due to hypothalamic or pituitary dysfunction may cause unopposed estrogen stimulation of the endometrium.
Eventually, the endometrial lining outgrows its blood supply and sheds irregularly, causing bleeding.
ودى زى حالة ال pco
- Coagulopathy Due to Liver Dysfunction
- Hypothyroidism (Secondary or Primary)
Can coexist with Wilson’s or result from pituitary dysfunction.
May cause abnormal uterine bleeding or irregular cycles, even in someone with otherwise absent menses.
حاجة مهمة و والأم فى الحالة دى قالتهالى أنها بدأت تاخد
Penicillamine والأم مقتنعة تمام أن هو دا السبب اللى عملها
Liver cirrhosis وباقى الأعراض
واعتقد دا خطأ الطبيب المعالج أنه مفهمهاش طبيعة المرض كويس وان ال penicillamine دوا موصوف لحالة بنتها
بس هل ممكن الدوا دا يكون سبب ال bleeding ؟
وفعلا دا طلع احتمال
If the patient has recently started on chelation therapy (like penicillamine), metabolic shifts might restore partial ovarian function leading to breakthrough bleeding.
السبب الأخير ( على حد البحث اللى وصلتله بس مش فى حالتنا دى )
- Iatrogenic Causes
Recent use of exogenous hormones (e.g., estrogen or progesterone for diagnostic or therapeutic purposes) can cause withdrawal or breakthrough bleeding.
بس دا بيكون لو الأم فى الأساس كانت بتدور على سبب ال amenorrhea ودا مكنش فى حالتنا
المعلومات ممكن تكون مش كاملة لأن ال Wilson مرض كبير ومعقد وأعراضه كتير وللأسف علشان هو autosomal recessive الحالة دى برضو اختها الأصغر اتشخصت بيه وباقى اخواتها مقدروس يعملوا التحليل لانه غالى (70ألف علشان يتعمل للأختين ) غير مصاريف العلاج ، كان الله فى العون
