🚨 ECLAMPSIA — What to do during the fit? إذا المريضة المصابة بـ Eclampsia دخلت بـ active convulsion / fit: 🛑 أولاً: ABC 🔹 Left lateral position 🔹 تأمين الـ Airway + suction عند الحاجة 🔹 Oxygen إذا كان عندها hypoxemia 🔹 IV access + قياس BP, SpO₂, blood glucose 🔹 لا تضع أي شيء داخل فم المريضة أثناء الـ seizure. 💉 Magnesium Sulfate = FIRST LINE ⭐ Loading dose: ➡️ MgSO₄ 4 g IV خلال 5–15 دقيقة ثم: ➡️ 1 g/hour IV infusion 🔴 إذا تكررت الـ seizure: ➡️ MgSO₄ 2 g IV إضافية خلال حوالي 5 دقائق. 💊 وين يجي دور Diazepam؟ ❌ Diazepam ليس first-line في Eclampsia. يُستخدم كـ alternative إذا كان Magnesium sulfate غير متوفر أو contraindicated. وفي حالة refractory seizure despite adequate MgSO₄ يمكن التفكير بالـ benzodiazepine مع استدعاء التخدير/العناية الحرجة. ⚠️ Diazepam قد يسبب: • Respiratory depression • Hypotension • Maternal sedation • Neonatal respiratory/CNS depression 🩺 بعد السيطرة على الـ fit 🔸 السيطرة على severe hypertension 🔸 مراقبة الأم والجنين 🔸 البحث عن complications 🔸 Delivery is the definitive treatment بعد استقرار الأم، وليس أثناء الـ active convulsion. Eclampsia + active fit → MgSO₄ FIRST, not Diazepam. MgSO₄ is superior to Diazepam for prevention of recurrent seizures and maternal outcomes.
