Treatment of Snake Bite(سم الثعبان) 😁
1-Reassure the patient
2-Remove tight items (e.g. rings, watches, or tight clothing) near the bite site
3-Immobilization of the bitten limb by splint to minimize spread of toxins → Crepe bandage + back slab
ممكن استخدام قطعه خشبيه للتثبيت
4- Make the bitten limb at the level of the heart, not up to avoid systemic spread or down (dangling) to avoid toxin accumulation in limb which leads to lymphatic obstruction
-مص وبصق الدم اثبت عدم فائدته
-إذا كان هناك رباط شديد على أو أعلى منطقة اللدغة يجب عدم إزالته بسرعة وإنما تدريجياً خلال دقيقة (يمكن استخدام الـ cuff حق الـ sphygmomanometer وتعمل cuff inflation لأعلى من الـ systolic BP وتعمل cuff deflation ببطء) لأنه لو أزيل الرباط فجأة يحصل اندفاع للـ toxin إلى الـ systemic circulation وتدهور حالة المريض
-يمكن ربط أعلى مكان اللدغة بـ crepe bandage بحيث تقدر تدخل اصبعك تحتها (ما يكون مشدود زيادة)
5-Antivenom
-Mild → 5 vials of snake venom antiserum in 500 cc NS over 1 hour
-Moderate → 10 vials of snake venom antiserum in 500 cc NS over 1 hour
→ تعاد الجرعة كل 6 ساعات لما يرجع الـ INR طبيعي
-Severe → 15 vials of snake venom antiserum in 500 cc NS over 1 hour. → تعاد الجرعة كل
ساعه ساعتين إلى أن يتوقف الـ bleeding ويتحسن الـ INR
-إذا الـ INR مرتفع والـ platelets ناقصة نعطي:
FFP 5–10 ml/kg
Platelets 4–6 units
إذا الـ INR مرتفع وال platelets طبيعي → FFP فقط
طيب كيف اقسمه الى mild, moderate, sever 👇
◾mild : , swelling localized at bite, bloody spots .
■moderate : swelling extend beyond site of bite and cross the nearest joint
asymptomatic thrombocytopnia and asymptomatic prolongation in INR, pt and PTT.
■severe :
#clinical bleeding ( DIC )
أكثر الحالات مواجهة في المناوبات هي النزفية .
#respiratory muscles weakness, extraocular muscles weakness ( diplopia ) , bulbar muscles weakness ( dysphagia )
in heavy intoxication : circulatory collapse.
acute renal failure
6-Anti-tetanus 1 ampule IM AST
+قبل إعطاء Snake Venom Antiserum يجهز:
Hydrocortisone 200 mg
Adrenaline 1 mg
Avil 10 mg تحسباً لحدوث حساسية
-جرعة الـ Snake Venom Antiserum هي نفسها في الـ Adult والـ Children
-لا يُجرى اختبار الحساسية قبل إعطاء الـ Snake Venom Antiserum
-إذا تحسس المريض من الـ Snake Venom Antiserum أو دخل في Shock ماعاد نعطية والمريض ممكن يتوفى !!
