🔴 Sodium Bicarbonate in severe metabolic acidosis! Consider this scenario: A 5-day-old preterm infant born at 30 weeks' gestation has required CPAP since birth with an FiO₂ of 0.25. He develops lethargy and frequent apnea and is treated empirically for presumed sepsis. He subsequently requires mechanical ventilation. Despite appropriate fluid resuscitation and treatment of sepsis, he develops severe metabolic acidosis and persistent oliguria. Arterial blood gas analysis shows: pH: 6.95, PaCO₂: 43 mmHg, PaO₂: 65 mmHg, and HCO₃⁻: 12 mmol/L. As a temporizing measure, 1 mEq (2 mL) of 4.2% NaHCO₃ is administered. Assuming an initial serum Na⁺ of 140 mEq/L and a normal intravascular volume, what is the expected rise in serum sodium concentration? The short answer: 7 mEq/L, from about 140 to 147 mEq/L. Why is this potentially dangerous? In an oliguric infant, the kidney cannot readily excrete the sodium load accompanying bicarbonate. A small dose (only 1 mEq in a 1-kg infant) can represent a substantial sodium load. Repeated doses could result in: Rapid hypernatremia and hyperosmolarity Worsening intracellular dehydration Additional electrolyte disturbances Unnecessary volume expansion, if repeated bicarbonate-containing fluids are administered Increased CO₂ generation, which can be problematic if ventilation is inadequate A paradoxical worsening of intracellular acidosis despite an improvement in measured extracellular pH Potential worsening of hypocalcemia, because alkalinization reduces ionized calcium Bottom line : In a severely acidemic, oliguric infant, always remember that giving NaHCO₃ means giving Na⁺ that the kidneys cannot readily excrete. If you’re interested in calculation, here it is. For a 1-kg preterm infant, estimated blood volume is approximately 100 mL/kg, or 0.1 L. A 4.2% NaHCO₃ solution contains 0.5 mEq of Na⁺/mL. Therefore: 2 mL × 0.5 mEq/mL=1 mEq Na+ Initially: 140 mEq/L×0.10 L=14 mEq Na+ After giving 1 mEq Na⁺: Na_new≈(14+1)/0.102≈147 mEq/L Thus, the estimated rise in serum sodium is ~7 mEq/L, from about 140 to 147 mEq/L. @newborn @newborn
