The anatomy of the anterior cranial fossa in patients with large frontal sinuses is particularly remarkable. In these cases, the fossa is elevated and displaced posteriorly. Consequently, the standard frontotemporal approach should be performed with an offset. This modification does not compromise the surgical exposure, while preserving the integrity of the frontal sinus eliminates the need for sinus reconstruction and prevents potential cerebrospinal fluid leaks.
