Nephrology Davidson and Harrisson.
Urinary tract obstruction is an important and potentially reversible cause of kidney failure. This patient is at risk for urinary obstruction based on her history of colon cancer. Although recent NSAID use may be contributing to the rapidity of her kidney damage, routine dosing is less likely to cause AKI in the absence of preexisiting renal dysfunction. Ultrasound of the kidneys is the best screening test for obstruction. Hydroureter and/or hydronephrosis may be found and suggest presence of obstruction. Although obstruction may be unilateral, it rarely causes clinically signi!cant renal failure in the absence of underlying renal disease. CT of the abdomen is useful after ultrasound to evaluate the site and etiology of obstruction. Postvoid residual is useful if functional causes of obstruction are suspected, such as urinary retention. After the obstruction site is located, retrograde urography with stent placement may be indicated, but only after de!ning the presence or absence of obstruction.
