Atrioventricular nodal reentrant tachycardia (AVNRT) is the most common form of sustained paroxysmal supraventricular tachycardia (PSVT). Other features of SVT may include constant RR-intervals and retrograde P waves. Patients typically present with palpitations, but may also have lightheadedness, shortness of breath, diaphoresis, chest pain, presyncope, or syncope. In hemodynamically stable patients, the next step in management is to attempt termination of the arrhythmia with vagal maneuvers (carotid sinus massage or Valsalva maneuver). Vagal maneuvers increase parasympathetic tone and temporarily slow the conduction and/or cause a transient AV nodal block, leading to termination of AV node-dependent arrhythmias, including AVNRT.
