Facilitating Real-World Advance Care Planning

Advance care planning (ACP) and serious illness communication guides complex medical care and is widely recommended in national guidelines, quality metrics, and Medicare policy, which has allowed physician reimbursement for ACP conversations since 2016. Yet prior research reveals disparities in serious illness care among Black individuals in the United States, and there is a paucity of research on the effectiveness of ACP interventions among them. Possible reasons for these disparities include, but are not limited to, inequitable access to health care and justifiable distrust in the health system.
