watershed areas of artery supply
اول مكان يصير بي necrosis هو ال
Distal part of transvers colon ( the splenic flexure )
لان هذا مكان متنازع بين ال inferior وال superior
هم اكو spectrum
يا يجي reversible colopathy
او transient colitis
يا يجي gangrene & fulminant pancolitis
Causes :-
- Arterial thromboembolism
- severe hypotension
- colonic volvulus
ال intestine تفتر على روحها والarteries تنبرم وتنسد
- strangulated hernia
- systemic vasculitis
- hypercoagulable states
- abdominal aortic aneurysm surgery —> Ischaemia of the descending & sigmoid colon
Clinical features :-
الكيسات اقل بهواي من ال superior
المشكله وين؟
من اريد اسويله colonoscopy لازم احضره بيوم يومين
واصلا ورا يومين تقريبا الcolon يرجع normal فادخل الكه ماكو شي
ويخربط عليه ال diagnosis
- elderly pt
presented with :
sudden onset of cramping
left-sided, lower abdominal pain
rectal bleeding
هاي الاعراض بيوم او يومين تروح ويرجع وضعه طبيعي
- Minority develop gangrene & peritonitis —> Resection
