Ischaemic gut injury
• Acute small bowel ischaemia :-
superior mesenteric artery
ينطي supply لل small intestine ولحد
distal part of transvere colon ( the splenic flexure of the large intestine )
يعني اذا انضرب هذا الارتري
ال absorption مال food كله يوكف
ياكل المريض بس ماكو absorption
( عكس ال chronic ال pt ما ياكل )
Causes :-
- embolus
يجي المريض
hypertensive
DM
AF
تمشي وتدخل بالارتري وتسده
Most common cause —> 50% of cases
- thrombosis
شلون ال MI توكف بنص الارتري
- hypotension
كلش قوي لدرجه ال blood ما يوصل لل gut
بسبب HF او Sudden blood loss او الخ…
- Vasculitis ( rare )
- venous occlusion ( rare )
ال portal vein ينسد فجأه ويصير congestion يكوم الارتري يذب blood والفين ما يستلم
Acute congestion —> developing perforation
- pt with AF + abdominal pain
اعتبره acute mesenteric ischemia لحد ما اسويله Ix كامله واتاكد هو acute mesenteric ischemia لو لا
- clinical spectrum
ياما يجي كلش pain ومتألم و necrosis وحتى gangrene
ياما اصلا المريض ما يدري بروحه
- pt have evidence of cardiac disease and arrhythmia
اذا اجه mcq على هذا الموضوع فكال اكيد احط كلمه arrhythmia بي
- عاده المريض يجي عنده الم قوي نفحصه نشوف no tenderness نفكر بي مأجوس ونطلعه
يجي ورا 6 ساعات صاير بي
Acute mesenteric ischemia
Presented with
peritonitis
WBC ⬆️
Leucocytosis
metabolic acidosis
hyperphosphataemia hyperamylasaemia
بس too late to do anything
50% pass
50% تنجح عمليتهم بس يصيرون short bowel syndrome
* انتبهوا ال hyperamylasaemia يصير بال pancreatitis وهم tenderness ضعيف
Ix :-
- X-rays —> thumb-printing due to mucosal oedema
- Mesenteric or CT angiography reveals an occluded or narrowed artery
Tx :-
- Resuscitation اول شي
- Mx of cardiac disease
- IV antibiotic
لان حتوقع يصير عنده perforation بينما يشتغل ال drug
- early intervention —> embolectomy and vascular reconstruction may salvage some small bowel
Or high surgical risk —> thrombolysis
- Small bowel transplantation
شي صعب مموجود حاليا
- اعالج underlying cause
مثلا AF انطي anticoagulants
- اذا تاخرت بالعلاج 75% mortality rate
اذا عاش يعاني من nutritional failure بسبب short bowel syndrome
وال quality of life تصير zero
