• Managemet of peptic ulcer
ليش اسوي managemet ؟
حتى :-
- relieve symptoms
- induce healing
- prevent recurrence
Eradication to All patients with ulcers who are H. pylori
يعني من اعالج المريض من القرحه ما اخلي يرجع مره ثانيه لان صارت بي قرحه
▪︎ لازم من البدايه انطي شي قوي واجنب ال Resistance
1) Triple therapy :-
PPI ثابت + two antibiotics ( clarithromycin ثابت plus amoxicillin or
metronidazole )
For 14 days
نسبه نجاح هذا العلاج 90% بالدول المتقدمه ( مو العراق )
بالعراق ما نستخدم هذا الدوا
الدول الي بيها
clarithromycin resistance more than 20% --> triple therapy ❌ ما استخدمه
بس ممكن انجبر على العلاج هذا

In pregnancy
In childhood
2) quadruple therapy :-
omeprazole (or another PPI),
bismuth subcitrate, metronidazole and tetracycline (OBMT)
for 10–14 days
مشكلته انو غالي
* اذا ما فادوا هذولي العلاجين ف يبقى على ال omeprazole for long-term
3) treatment guided by antimicrobial sensitivity testing
4) rescue therapy (levofloxacin, PPI and clarithromycin)
5) long-term acid suppression
بالعراق امشي بهيج ترتيب بالعلاج :-
quadruple therapy
ما فاد
levofloxacin + PPI + amoxicillin
ما فاد
PPI + amoxicillin + rifabutin

▪︎اكوله اكطع الNSAIDs
الPt الي لازم يبقى الى ال NSAIDs هذا لازم اسويله eradication therapy حتى اقلل الrisk مال ulcer
Pt with long-term NSAID therapy should first undergo eradication therapy to reduce ulcer risk
مثلا MI وياخذ large dose of aspirin هذا انطي PPI وياه
اذا low dose مو كلش ضروري ال co-prescription of PPI + NSAID
اني اعرف ال non-selective NSAID تشتغل على الcox-1 وال cox-2 وقويه وتسويلي bleeding
من انطي للمريض selective NSAID كأنما منطيته
PPI + non-selective NSAID
▪︎ جزء ضروري من ال managemet هو ال life style modification
اكول للpt اكطع التدخين
Smoking --> ⬇️ eradication rate, ⬆️ ulceration
Alcohol in modration ما يسوي شي
بس ال alcoholic person الي يشرب بشكل غير معقول وجره وحده high concentration هو الي يصير عنده
▪︎ ال surgical treatmeant كلش rare لل peptic ulcer
لان بي هواي complications
▪︎no special dietary advice is required
Eradication of the infection has proven benefits in several extragastric disorders :-
1) unexplained B12 deficiency
2) unexplained iron deficiency anemia
3) idiopathic thrombocytopenic purpura
Platelet counts improve and may normalize after eradication therapy
