بخش دوم: نمونه هایی از دستورات استاندارد در بخش اورژانس

در این بخش، نمونه‌هایی از دستورات استاندارد (Standard Orders) آورده می‌شود. توجه داشته باشید که این موارد صرفاً به‌منظور آموزش ساختار و ترتیب اصولی دستورنویسی تنظیم شده‌اند. این دستورات، دستورات واقعی بالینی نیستند و ارتباطی میان تشخیص‌ها و دستورات درج‌شده وجود ندارد. هدف صرفاً آشنایی با نحوه‌ی صحیح نوشتن، ترتیب منطقی بخش‌ها، و شکل حرفه‌ای تنظیم دستورات در محیط اورژانس است.

👉 GENERAL ORDERS

Impression: Acute Cholecystitis with dehydration.
Condition: Urgent.
Position: Semi-sitting.
Activity: Complete bed rest with side rails up; fall precautions.
Diet: NPO until surgical evaluation.

👉 MONITORING

- Monitor BP, HR, RR, SpO₂ continuously.
- Record vitals q15min × 4, then q1h.
- Cardiac Monitoring.
- input/output chart.

👉 OXYGEN / AIRWAY

- O₂ via nasal cannula 2 L/min.
- If SpO₂ < 90% → use simple mask 6 L/min.
- Intubation preparation
- suction ready at bedside.

👉 IV ACCESS / FLUIDS

- Insert 18G IV line ×2.
- NS 1000 mL IV in 4 hours.
- Keep second line patent with NS lock.
- If signs of fluid overload Discontinue fluids.

👉 BLOOD PRODUCTS / TRANSFUSION

- Transfusion of 1 unit of crossed match PRBCs

👉 INVESTIGATIONS

- CBC, BUN, Cr, Na, K, Glucose, LFT, Amylase, Lipase.
- PT/INR, aPTT, Troponin I.
- ABG if SpO₂ < 90%.
- Crossmatch 2 units PRBC if indicated.
- Blood culture ×2 before antibiotics.
- Urine analysis and culture.
- ECG now.
- CXR (PA view).
- Abdominal ultrasound (RUQ pain – rule out cholecystitis).
- Abdominal CT with contrast after BUN,Cr results.
- Point-of-care glucose.
- Serum lactate
- Bedside E-FAST

👉 MEDICATIONS

- Epinephrine 0.3 mg IM stat
- Atropine 1 mg IV stat (if HR < 40 bpm).
- Nitroglycerin 0.4 mg SL stat ( if SBP > 90).
- Norepinephrine infusion 5 μg/min (if SBP < 90).
- Ceftriaxone 1 g IV stat and q12h.
- Metronidazole 500 mg IV stat and q8h.
- Pantoprazole 40 mg IV q12h.
- Enoxaparin 40 mg SC stat and daily.
- Paracetamol 500 mg PO PRN for T > 38°C.
- Ondansetron 4 mg IV stat.
- Morphine 3 mg IV stat if Pain score>6

👉 NURSING / SUPPORTIVE CARE

- Insert Foley catheter; monitor urine output hourly.
- Maintain IV patency and monitor infusion site.
- NPO until further notice.
- Maintain patient warmth and comfort.
- Elevate head of bed 30°.
- Record pain score q2h.
- Be cautious for any hypersensitivity reactions.

👉 CONSULTATIONS

- Surgery for evaluation and management.
- Internal Medicine – for comorbidity management.
- Radiology – for urgent imaging coordination.

👉 DISPOSITION / PLAN

- Admit to General Surgery Ward once stable.
- Reassess after imaging and labs.
- To be discharged if all investigations are negative and symptoms have resolved.
