This increases impingement during arm elevation. Inflammation and tendon degeneration Repeated impingement causes inflammation of the bursa and tendons. # Chronic cases may progress to tendinopathy or partial/full-thickness rotator cuff tears. Painful Arc 0–60°: Usually little or no pain. 60–120°: Painful arc due to maximum impingement. 120–180°: Pain often decreases because the greater tuberosity passes beneath the acromion and the compressed tissues are relieved.Exercise Progression Criteria Progress only when: Pain ≤2/10 Full ROM No painful arc Scapular dyskinesis corrected External rotation strength ≥80% of opposite side Avoid ❌ Empty Can in early painful stage ❌ Heavy overhead press ❌ Behind-the-neck press ❌ Upright row above shoulder height ❌ Repetitive painful abduction #physiotherapy#physiotherapy_world https://www.instagram.com/reel/Db1j1WmT3Oa/?igsh=MWswN25ndjJuZHRoMA== 1.3K·Aug 9 https://www.instagram.com/stories/physio_._com/3945788473412403502?utm_source=ig_story_item_share&igsh=MWgzemY2czhpcDNycA== 1.6K·Jul 21 The one who needs observership Dm me on instagram 10.5K·May 28 De Quervain Tenosynovitis – Description Definition De Quervain tenosynovitis is an inflammatory condition affecting the tendons of the first dorsal compartment of the wrist: Abductor Pollicis Longus (APL) Extensor Pollicis Brevis (EPB) These tendons pass through a fibro-osseous tunnel near the radial styloid process. Repetitive thumb and wrist movements cause friction, thickening of the tendon sheath, pain, and restricted motion. Biomechanics (Reference: Cynthia Norkin & Kisner) Normal Biomechanics The thumb performs: Abduction Extension Opposition Gripping and pinching activities APL and EPB stabilize and move the thumb during: Grasp Lifting Twisting Ulnar deviation of wrist Pathomechanics Repeated: Thumb abduction Thumb extension Wrist ulnar deviation increase tendon friction inside the first dorsal compartment, leading to: Synovial sheath irritation Tendon swelling Pain during movement Reduced grip strength Common in: Repetitive hand workers Mobile overuse New mothers lifting babies Typing and household activities Clinical Features Pain over radial side of wrist Tenderness near radial styloid Swelling Pain during thumb movement Weak grip strength Difficulty holding objects Positive Finkelstein test Physiotherapy Treatment Protocol Acute Phase (0–2 weeks) Goals Reduce pain and inflammation Protect tendons Maintain ROM Treatment Thumb spica splint Cryotherapy Activity modification Gentle pain-free ROM Electrotherapy if needed Exercises Thumb AROM Wrist ROM Tendon gliding Isometric thumb exercises Subacute Phase (2–4 weeks) Goals Improve mobility Begin strengthening Treatment Soft tissue mobilization Stretching Light strengthening Exercises Towel twisting Radial/ulnar deviation Putty squeezing Ball squeezing Pinch strengthening Rehabilitation Phase (4–6 weeks) Goals Restore strength and function Improve endurance Exercises Theraband wrist strengthening Thumb extension strengthening Functional grasp training Weight-bearing exercises Advanced Phase (>6 weeks) Goals Return to work/sport Prevent recurrence Exercises Advanced resistance training Functional task simulation Ergonomic correction Work-specific hand activities Important Exercises Included in Poste https://www.instagram.com/reel/DY3raRisv6U/?igsh=NGFzdWNsejZtOXV6 2.8K·May 28
