Inferior STEMI + Right ventricular infarct RVMI + CHB
this pt has BP 195/115 .... shall we lower BP or not???
answer is:
1) exclude airtic dissection which can extends to right coronary artery and cause such features.
2) if aortic dissection was excluded, then no anti hypertensive should be given, since hypotension is very common, and even small doses of anti-HTN medication can result in shok.
shock state will further result in coronary hypoperfusion 👉👉 more infarction 👉👉 worsening of shock
this is very practical point, commonly missed
