A nurse, young age, female
-ve past medical and surgical hx.
she with post prandial pain + vomiting for more than 7 mn ... which make her to avoid oral intake .... and lost significant weight.
extensive medical workup was -ve including: CBC, LFT, RFT, TSH, S cortisol, s electrolytes, lipase, amylase, abd US, multiple OGDs, and many trials of PPI.
brain MRI + ophthalmological review were normal.

now, the patient presented with weakness .
inx showed hypokalemia and hypo Mg

طبعا بديت وياها الطريقة الي حاليا الكل تعود عليها:
NS ...
thiamine
phosphate replacement
KCl + MgSO4 replacement

sent for PO4, Cu, albumin

و ثاني يوم كتبتلها على

CT mesenteric angiography
و بالهستري كتبت
to exclude SMA syndrome vs Peritoneal Cocoon vs partial intestinal obstruction of Foramen of Winslow

كتبتهم كلهم رير، لأن ما معقولة هيج هستري و الف مراجعة و اني اكتبله مثلا
acute pancreatitis

و هذا كلش يفيد ابو الاشعة، حتى اخليه يعرف بشنو جاي افكر انا.

طبعا الفضل بسرعة التشخيص يعود للزملاء السبقوني و الوورك اب مالهم ... بحيث انا بديت من وين ما هم انتهوا.

و انتهت معاناة مريضة شابة أيست من الحياة و فقدت الأمل بالتشخيص
